It's not just the head size to body ratio, it's the classic dwarfism look hes got goin there.

There is a difference between a dwarf and a small person, although Shorten would have to be at least 5'6" plus. Shorten's body is evenly proportioned whereas a dwarf's isn't. It's only the shape of his head and large forehead which makes us think of dwarfism.
