Let's say you were born with a hole in your heart. You would get out of breath easily, be less able to tolerate exertion and have to sit down and rest a bit more than the average person. Not too much, but let's say over an average day you might sit for an hour longer than an average person.

People's appetite reflects their level of physical exertion. Many of us do maybe 10% of the physical work our ancestors did. You do not need to push your heart to get exercise.
