Why is it so hard to get a decent national health care system for the US?

In the mid-90's Taiwan decided to create a healthcare system from scratch and looked around the world for what worked and what didn't and was successful in that endeavour. Obama on the other hand promised a single-payer healthcare system as a candidate but now has taken that idea off the table because health insurers and big pharma has the US Congress in their pocket - the US medical-industrial complex won't allow it.
